There is a standard medical form to complete
as part of every PADI course or experience.

the guidelines in this section, and answer "Yes" or "No" to
the questions laid out. If you answer "Yes" to
any of the questions in this section, it
does not necessarily prevent you from diving

Fit to dive medicals if a YES box has been
ticked on a PADI/BSAC pre-diving medical

Fit to return to diving after treatment
for DCI
AS4005.1 for diving in Australia
Fitness to return to diving after DCI
Fitness to dive, if there is a medical
problem e.g. asthma
Any problem from equalizing to "before
I pay for a course am I fit enough?"
